Physiotherapy MSK SELF-REFERRAL FORM Belfast Health and Social Care Trust

Self-referral is available for adults over 16 who need support and advice to manage symptoms related to muscle strains/joint sprains/back and neck pain.

This referral option is not available if you require a home-based appointment or if you are seeking treatment for a neurological / respiratory / continence condition. If you have a pregnancy-related pain please ask your GP/Midwife to direct you to the appropriate service.

If you are referring for headaches or falls, you must have had a consultation with your GP prior to referring.

Podiatry Referrals.

What the service does

Podiatrists assess, diagnose and treat people with problems that affect their:

feet
ankles
lower limbs

The service includes:

nail surgery
managing foot-related problems for patients with the following conditions:
- diabetes
- vascular disease
- cancer
- arthritis and other disorders of the joints, muscles and ligaments
- kidney disease

treating mechanical foot problems with special appliances or insoles
Integrated Clinical Assessment and Treatment Service (ICATS)

Feet, ankle and lower limb problems can be caused by a range of conditions, such as:

bone and joint disorders, for example arthritis
soft tissue and muscular pathologies
neurological and circulatory diseases
foot injuries and infections caused by sports and other activities

Who provides the service

We have podiatrists working in hospital and community settings. Hospital podiatrists work with a range of other services and manage foot problems related to acute complex conditions.

Community podiatrists provide a comprehensive foot health service in health centres.

How to access the service

In hospital, you can be referred to the podiatry service by medical staff for assessment and advice.

In the community, you can contact the service directly through the referral numbers listed below. You can also ask a GP, family member or other healthcare professional to do it for you.

Test Results.

xray Please phone after 4pm to enquire about your test results as our reception staff will have more time to deal with your request at this time.

Note that the practice has a strict policy regarding confidentiality and data protection and we will only release test results to the person to whom they relate unless that person has given prior permission for the release of this data or they are not capable of understanding the results.

When you take your test you will be told how long it will be before the results are returned to the practice.

It is your responsibility to check your results and to make an appointment to discuss them with your doctor if your are advised to do so.
